Worker's Compensation Forms for New York
The New York State Worker's Compensation Board indicated
that the new worker's compensation forms must be used as of April 1, 2009. The new
C4 form is easier to read. There is more white space between
questions, more space to answer questions, and a larger type
face to make the new forms easier on your eyes. Microwize Technology is pleased to offer the new worker's
